CONSENT FORM OF THE DATA SUBJECT
I have read this Consent Form and I hereby accept, declare, undertake and consent in advance to the collection, recording and processing of my personal data by Op. Dr. Şerif Yavuz in accordance with the Law No. 6698 on the Protection of Personal Data.
In accordance with the “Law on the Protection of Personal Data” No. 6698 and the “Regulation on the Processing of Personal Health Data and Ensuring Privacy”, I hereby agree, declare, undertake and consent that my personal data may be collected, recorded and processed verbally / in writing by Op. Dr. Şerif Yavuz. Şerif Yavuz to process my personal data, sensitive personal data and health data, including any personal data identifying or identifying my identity given by me verbally / in writing and/or electronically; to share with other persons and organizations where sharing is required within the scope of the relevant legislation; to process all kinds of personal data, sensitive personal data and health data detailed below within the scope defined in the “Law No. 6698 on the Protection of Personal Data” and “Regulation on the Processing of Personal Health Data and Ensuring Privacy”.
Documents such as driver’s license, identity card and passport, tax number, health insurance number, signature information, license plate, etc. containing information about the identity of the person; name-surname, identity/passport number, nationality information, mother’s name father’s name, place of birth, date of birth, gender, which are clearly belonging to an identified or identifiable natural person; processed partially or completely, automatically or non-automatically as part of the data recording system.
Information such as telephone number, address, e-mail address, fax number, IP address, which clearly belongs to an identified or identifiable natural person; is processed partially or completely automatically or non-automatically as part of the data recording system.
Personal data relating to records and documents taken at the entrance to the physical space, during the time spent in the physical space; camera records, fingerprint records and records taken at the security point, which clearly belong to an identified or identifiable natural person; processed partially or completely automatically or non-automatically as part of the data recording system.
Personal data clearly belonging to an identified or identifiable natural person; photographs and camera records (except for records falling within the scope of Physical Space Security Information), data contained in documents that are copies of documents containing personal data.
Data that clearly belongs to an identified or identifiable natural person; processed partially or completely automatically or non-automatically as part of the data recording system; data specified in Article 6 of the Law on the Protection of Personal Data (e.g. all kinds of health data including blood group, biometric data, etc.).
Personal data clearly belonging to an identified or identifiable natural person; processed partially or completely, automatically or non-automatically as part of the data recording system; Personal data regarding the receipt and evaluation of any requests or complaints addressed to Op. Dr. Şerif Yavuz.
